Inter-Agency Specialist - APD

  • Job Summary:

    This is a professional position responsible for the coordination and facilitation of requirements for children in the child welfare system or at risk of entering the child welfare system that may meet the criteria for services with the Agency for Persons with Disabilities (APD), or other agencies/providers.

  • Essential Functions:

    • Establish and maintain positive working relationships with Heartland for Children internal and external customers
    • Educate and communicate with case managers, caregivers and other professionals on the APD process
    • Review psychological assessments to determine if youth has a qualifying APD diagnosis
    • Collect, review records and submit APD applications for youth with qualifying APD diagnoses 
    • Provide assistance, guidance and direction on completing forms such as waiver enrollment, waiver support coordination selection, annual status reviews, and release of information forms
    • Coordinate QSI assessments between case management and APD
    • Request for Administrative Reunification, follow up and track to ensure process is completed timely
    • Process eligibility determinations from APD, notify case management, and assist with appeal process if needed
    • Submit APD crisis requests when youth meets the criteria for a crisis placement Communicate with APD providers regarding acceptance of children into their APD group homes, by coordinating conference calls, gathering records, and communication with HFC contracts department regarding payment, etc.
    • Review all APD clients/pending clients in FSFN, review monthly work/progress on youth and update APD spreadsheet.
    • Coordinate and facilitate monthly meeting with APD/DCF to review clients and pending clients.
    • Participate in DCF and MDT staffings regarding APD clients or potential APD clients as needed and complete follow up tasks.
    • Communicate with APD providers regarding payment/services issues. 
    • Assist in arranging testing and coordinating ABA services for children for APD purposes
    • Facilitate MDT staffings for clients pending or approved for APD services
    • Coordinate APD residential transitions for youth
    • Understand  and manage the network of applied behavior analysis (ABA) provider and services
    • Serve as a back-up and assist the Inter Agency Specialist with community mental health, substance abuse programs and dually involved youth with the Department of Juvenile Justice
    • Perform special projects as assigned

  • Qualifications Required:

    Bachelor’s Degree in Social Work or Human Services field.  Minimum of three years' experience in child welfare or related field. 

     

  • Qualifications Preferred:

    Knowledge of development disabilities required  Experience working in FSFN preferred.
